When you sign up for your first appointment, we’ll email you some intake forms to complete online. However, if you use paper forms, you can print these and bring them to your first session.

We are happy to work with other providers to ensure we take a holistic approach to your health and well-being. In order to make this work in accordance with HIPAA guidelines, we’ll need you to sign an authorization to disclose information. This enables us to share information with your team.

We accept Aetna, BCBS, cash, checks, major credit cards, HSA, FSA, and Victims of Crime as payment for our services.
Each person experiences grief and trauma differently and we do not believe this is a disorder. We provide ourselves on providing personalized services tailed to meet your needs without the assumption that what you’re going through is an illness that needs to be cured.
